White matter hyperintensities (WMH) are bright spots visible on magnetic resonance imaging (MRI) scans of the brain, typically found in the white matter. They indicate areas of altered white matter integrity and are associated with increased risk of atrophy, especially in the hippocampus and precuneus regions. WMH are often studied in the context of Alzheimer’s disease, dementia, and the effects of resistance training on brain health.

What this means in real life

A 65-year-old with high blood pressure gets an MRI for memory concerns; the scan shows white matter hyperintensities, which may explain some cognitive slowness and increase monitoring for future stroke risk.

What it isn’t

These are not tumors, infections, or always a sign of serious disease. Many people with white matter hyperintensities have no symptoms, and their presence alone does not diagnose dementia or guarantee cognitive problems.

Commonly misused online

Social media often conflates white matter hyperintensities with dementia or Alzheimer's disease, implying they are always pathological; in reality, they are common in aging and often clinically silent.
