'Undruggable' describes a protein or molecular target that cannot be effectively inhibited by existing drugs due to its structural properties, such as lacking a binding pocket or being too flexible. In oncology, many mutated proteins like KRAS were historically considered undruggable, meaning no therapeutic strategy could block their oncogenic activity. However, recent drug development has created inhibitors that successfully target these once-inaccessible targets.

What this means in real life

For decades, the KRAS protein—mutated in many cancers—was considered undruggable because it had no obvious pocket for drugs to bind to. Researchers eventually developed new approaches to target it indirectly, but it exemplified the frustration of pursuing a medically important target that resisted traditional drug design.

What it isn’t

It does not mean a disease cannot be treated at all, nor does it mean the target is unimportant. Many 'undruggable' targets are now being successfully targeted through novel approaches like protein degradation or allosteric modulation, proving the label is about current technical limitations, not absolute impossibility.

Commonly misused online

Social media often uses 'undruggable' to mean 'incurable' or 'untreatable,' implying hopelessness. In reality, it refers only to a specific technical challenge in drug design; many undruggable targets are now yielding to new therapeutic strategies.
